How To Complete The Application
Click the Download link for the application that you will need to complete.
The Application will open in a new browser window.
Complete the application in full by filling in each required form field.
Next the form will require you to Sign and then enter your email address.
Next click "Click To Sign."
Once you have done so Adobe will send you a confirmation email to make sure you are the person that signed the document. Once you locate the verification email click the link to verify that you are the person who electronically signed the document.
Then it will send you a second email with your downloadable PDF of the completed application.
After downloading the completed application, if a notary is required you may have the document notarized.
You will only need to have documents notarized if the document states it.
Once the document has been notarized you can submit it via email to the board.
Submit your completed applications to: firstname.lastname@example.org
If you would like to have your documents notarized online you may choose any company that will notarize the document for you.
